Intraoperative parathyroid hormone levels measured by intact and whole parathyroid hormone assays in patients with graves’ disea
Fri, 02/29/2008 - 08:42Abstract
Purpose To find out if the whole parathyroid hormone (wPTH) assay has practical advantages over the intact (iPTH) assay in patients
with Graves’ disease.
Methods We measured iPTH and wPTH levels before and after subtotal thyroidectomy in 111 consecutive patients (94 women and 17 men)
with Graves’ disease. Blood samples for assays were obtained after the induction of anesthesia (basal) and following skin
closure (postoperative).
Propofol decreases early postoperative nausea and vomiting in patients undergoing thyroid and parathyroid operations
Thu, 02/28/2008 - 14:51Abstract
Background As the practice of parathyroid and thyroid surgery shifts toward short stay and outpatient treatment, the occurrence and management
of postoperative nausea and vomiting (PONV) increases in importance due to its potential to delay discharge. PONV also may
contribute negatively to the patient’s experience and thus their level of satisfaction. The purpose of this study was to determine
whether anesthetic technique based on propofol decreases the incidence of PONV and, consequently, improves patient satisfaction
with their care.
Minimally invasive video-assisted thyroidectomy for benign thyroid disease: an evidence-based review
Thu, 02/28/2008 - 14:51Abstract
Background A decade after nearly all surgical disciplines developed minimally invasive techniques, the first report of a single case
of minimally invasive thyroidectomy was published. Minimally invasive video-assisted thyroidectomy (MIVAT) is now considered
the most widely practiced and most easily reproducible minimally invasive procedure for thyroidectomy. The aim of this review
was to analyze the treatment of benign thyroid diseases by MIVAT.
Treatment and prevention of recurrence of multinodular goiter: an evidence-based review of the literature
Thu, 02/28/2008 - 14:51Abstract
Background Reportedly, 10−15% of patients with goiters ultimately require operative intervention, and recurrences of multinodular goiter
(MNG) account for up to 12% of all thyroid operations.
Methods We performed an evidence-based review of articles published in the English language between January 1987 and October 2007
relevant to the subject.
Results Medical treatment with T4 appears to be associated with a greater proportion of patients whose nodules decreased in size by
Wegener’s granulomatosis in the thyroid mimicking a tumour
Thu, 02/28/2008 - 14:35Abstract Wegener’s granulomatosis (WG) is a systemic vasculitis characterised by the presence of necrotizing granulomas and classically
manifests as a triad of upper and lower respiratory tract involvement along with glomerulonephritis. Other rather unusual
presentations of WG include ocular, salivary gland, cutaneous, gastrointestinal and cardiac involvement. We report a case
in a 51-year-old woman suffering from WG with positive antineutrophil cytoplasmic autoantibodies and antibodies directed against
The delphian lymph node in thyroid cancer.
Thu, 02/28/2008 - 07:49The significance of the Delphian lymph node (DLN) in thyroid cancer was examined in patients undergoing total thyroidectomy. The clinical appearance of the DLN is not an accurate indicator of the presence of unsuspected thyroid cancer. On the other hand, metastatic involvement of the DLN is an adverse prognostic marker with a significant correlation between DLN involvement, tumor size, and extensive lateral lymph node metastases.Page: 477DOI: 10.1097/SLA.0b013e31815efdc4Authors: Isaacs, Joseph D. Recommendation for subclass evaluation of tnm stage iva papillary thyroid carcinomas: t4an1b patients are at risk for recurrence
Tue, 02/26/2008 - 14:46Abstract
Background Although all tumor, node, metastasis system (TNM) stage IVA papillary thyroid carcinomas (PTCs) do not seem to behave equivalently
as a result of various tumor and node stages, to our knowledge, subclass evaluation has never been attempted.
Methods We reviewed 119 stage IVA PTC patients who underwent initial thyroidectomy with modified neck dissection as curative surgery
at our institution (33 male patients, 86 female patients; age 61.6 years; follow-up 87.7 months). These patients were divided
Intrathyroidal hematopoiesis: a rare histological finding in an otherwise healthy patient and review of the literature
Tue, 02/26/2008 - 14:43Abstract
Background and aims Multinodular goiter represents the most common thyroid disease in Europe and the United States. Regressive changes like hemorrhage,
infarction or fibrosis are frequently observed. Calcifications have also been detected by microscopic investigation of thyroid
tissue. Extramedullary hematopoiesis has been described in almost every organ, especially in patients with hematologic diseases.
However, true bone formation is uncommon and to date only two cases of metaplastic bone formation with extramedullary hematopoiesis
